I *finally* found out how to get a working NetworkManager and dnsmasq
back.  You have to remove *everything* from /etc/network/interfaces
except the loopback interface.  If there is anything else configured in
there then Network Manager doesn't handle network setup and, as a
consequence, doesn't run dnsmasq.

Incredibly simple but no one suggested this to me, I finally twigged
when looking at /etc/network/interfaces on the system where everything
is working as normal.
